Hi Akbar,
First check Phone url parameters in Enterprise Parameters Configurations.
Second if you are using name in urls insted of IP check dns configuration and acces for IP phones.

Dear friend
There is a checkbox on the DN Configuration page: Log missed calls. Is it unchecked on the line appearences where this isn't happening?.Other thing restart the TFTP server , if this is not working kindly restart it using hardware reset.
Thank you

Dear All Friends,
Thanks for your Feedback.
1) On Phone url parameters in Enterprise Parameters Configurations.
present configuration as below : with ip address DNS and Domain is configure as well.
http://10.x.x.x:8080/ccmcip/authenticate.jsp
http://10.x.x.x:8080/ccmcip/xmldirectory.jsp
2) not using LDAP synchronisation in call manager .
3) Already checkbox on the DN Configuration page: Log missed calls.checked on the line appearences.
4) yes this happens for on all phones ? how can i reset to all phone to factory.
So is there no way to reset the phone without reloading the whole thing?
Still not able get the Call Details record.
Regards
Syed

This is likely a security by default issue.
Please check the following cisco document,
https://supportforums.cisco.com/docs/DOC-17679
Please also check your IP Phone firmware updated correctly to new version with your 7.x to 9.x upgrade, you may need to go through a 2 step phone firmware upgrade process to get them to current version that came with 9.x.
