Just upgrade to 11.5 in UCM. (UCM PUB/2 UCM SUBS, UCON PUB/UCON SUB and Presence PUB/SUB)
Since upgrade Plantronics CS 540 with APC-43 No ring tones are detected in Headset.

I had same issue with some models after CUCM upgrade and we fixed by upgrading phone firmware versions.
