This call actually was setup from the ISDN Q.931 perspective. Something else failed, possibly the fax negotiation.

We would need a lot more information to help you.

Here is a fax troubleshooting guide.

http://www.cisco.com/en/US/docs/ios/12_3/vvf_c/voice_troubleshooting/old/vts_fax.html

I would start by attaching a handset to the FXS port and making sure that it can complete calls.

Assuming that works, then you need to look at fax negotiation between the PRI and FXS. Is it fax-relay, fax-passthru, or T.38? Are the ports H.323 or MGCP?

A "show tech" from the router will help immensely.

I have seen several fax cases with locally hairpinned calls resolved by "no local-bypass" under the voice-card.

If these tips and doc does not help, then please gather a "show tech" and we can go from there. A "debug voip ccapi inout" might also be helpful.
