RTMT Hardware Failure alert not triggering

We are running version 10.5 on Cisco C220 servers running ESXi 5.0.0.

 

We had a hard drive in one of the C220s fail over the weekend.  No one got an email from RTMT.  We only found out because of an audible alarm that the C220 was making.  

 

We have several UCM servers running on this particular C220 ESX host, including a publisher, subscriber, and Presence publisher.  I checked RTMT.  The Hardware Failure alert is enabled and set up with the correct trigger - on every poll and non-stop monitoring.  It is set up to email a distribution list and is using the correct mail server to send mail.  We frequently get alerts from RTMT, and each is sending to the same dist list using the same mail server, so no configuration difference there.  Also noticed that under Last Alert Raised for Hardware Failure, that no date displays.  Looks like the alert was never triggered.

 

Trying to find out how to fix this.  What do I need to do either in RTMT, the UCM, or the ESX host itself to make sure we have these alerts working.
