From sip debug point of view all is looking fine.

Can you share the output of "debug ccsip all" when the phone tries to register?

also the "show voice register pool all brief"

MAC/IP address of the phone.

Check if the register pool includes the IP address of the SIP Phone (check subnet mask).

If not there is a case where SIP phones when they belong in different subnet than CME they should use user/pass.

If this is the case use authenticate register command under voice register pool.

Also check

voice service voip
ip address trusted list ....

You may try erasing CTL files under phone security settings
