Unable to log into CCMUser page

I am doing a new install of CUCM 7.1. I have LDAP setup and a couple of phones associated with my new CUCM environment. I also have a user associated with each phone. When I go to the ccmuser login page, we enter our AD account info and it acts as if it is logging in, but then it just takes us right back to the login page. It will never log us in. Periodically, instead of taking my back to the login page, I this error page saying...

HTTP Status 400 - Invalid direct reference to form login page

type: Status report

message: Invalid direct reference to form login page

description: The request sent by the client was syntactically incorrect (Invalid direct reference to form login page).

Any ideas? Thanks!

What if you tried to set the password for that user in CUCM to something new and then set it to be changed at next login in the edit credentials?  Also make sure the user belongs to the CCMusers group.
