It doesn't appear to be available on software.cisco.com - and the notice is that it's End of Life - I would recommend asking TAC for a copy - they might have access to all the versions
I don't know what version of ISE that CM module was used - maybe you can install a fresh version of older ISE and see if it's still in there. I had a look in ISE 3.4 and it's only the newer versions .
