Operations Manager complains that I have a bad extension. For example, in the following case it says 3431 is an invalid extension:

3429:0004F2E02F12:10.195.18.19,

3431:00E07524A784:10.208.52.29

I fixed my own problem, you can't have a carriage return between phone entries.
