The most common causes of devices winding up in Questioned is that they are not ICMP reachable, or you are using SNMPv3 and the SNMP engineID is not unique across all devices. If LMS is running on Windows, make sure you explicitly allow ICMP echo requests out and echo replies in within Windows Firewall (or disable Windows Firewall). If the problem is the engineID, fix it so the engineID is unique across all devices, reboot them, and then delete and re-add them.
