I have QPM 4.1.5 installed. Here is simplifed example of QoS config on a router.

In QoS Report Card I can see only 3 classes.

There are no classes from LEVEL3 service policies. Is that QPM discovery limitation? Do you have experience with this?

Is this occurring on all devices? What kind of device is having the problem? QPM limitation is 12 classes per interface so you could be running into a bug. Try running a device update from Common Services --> Software Center --> Device Update and update all the packages.
Post the show ver of the device having this problem and a screen shot of the Common Services --> Software Center --> Software Update page.

Just on Cisco 3845 that has level 3 policies applied
