I've blocked these sites:
googlevideo.com
youtube-nocookie.com
youtube-ui.l.google.com
youtube.com
ytimg.com
ytmg.com
However the Youtube App still works. If I block the following sites, the Youtube app stops working but it also breaks Google Play Music which I want to keep.
ytimg.l.google.com
ytstatic.l.google.com
Is it possible to block youtube and the youtube app without also blocking Google Play music?

I believe that ytmg.com has nothing to do with Google/YouTube, so you may be able to remove it.
What you want to achieve is quite difficult if Google shares domains for their different services. See also this KB article. And a search of the forum reveals even more. I'm afraid you have to find it out yourself.

After much experimentation, I found that the following combination seems to work to block YouTube and the YouTube app for both Android and iOS, but this does not break Google Play music:
googlevideo.com
s.ytimg.com
youtube-ui.l.google.com
youtube.l.google.com
ytimg.com
ytmg.com
Also in the category list, block "Video Sharing"
If you have a dual band router, you can set one band up to use OpenDNS with these controls, and the other to use another DNS server or a different OpenDNS "Personal network" in your OpenDNS dashboard.
I hope this works for others! Try this and please do let me know how if it works for you.

Again, is it a mistake that ytmg.com went into your list? This doesn't seem to be related to Google/YouTube.

This worked for me:
Block: youtube.com
Whitelist: googlevideo.com, accounts.youtube.com, music.youtube.com
(I'm using NextDNS, so YMMV)
