1760 + IVR disconnect problem

First of all I want to tell you a bit about the system that i get the error message:

A 1760 with FXOs. Calling party redirected to DN YYY (CTI DN) to IVR according to plar configuration under the FXO ports and then if no internal DN entered by the caller at the end of the IVR message he/she will be redirected to operator DN (let it be XXX). So there is voice translation-rule, voice translation-profile and also translation-rule already configured.

And now a bit about the error message:

IVR can be listened with no problem.Everything is OK till the end of the message. As we expect the system should redirect the caller to the operator DN in case of no internal DN entered by caller. But at this stage of the process the call is terminated by the router with no notification.

Problem itself:

Only "debug voip ccapi inout" command's output gives out details of what really happened. And the only message it prints out at the end of the IVR message is ....

.../CCAPI/ccCallDisconnect:

Cause Value=16, Call Entry (Responsed=TRUE,CauseValue=16)

.../CCAPI/cc_api_get_transfer_info:

Transfer Number Is Null

.../CCAPI/ccCallDisconnect:

Any idea about "disconnect cause=16". What may it mean, or which points should be checked once more?
