Is there a way within unity or Call Manager to have a customer dial up the VM system, hit a key, get prompted for an extension or access code and then based on the extension or access code forward that call to an external number?

You can easily do this with either Unity or with CRA (JTAPI IP-IVR).

I am a little unclear on how to do it with Unity. Any help would be appreciated.

Create a Call Handler for this purpose with Transfer instructions to the number to be transferred to, and then set the Caller Input on your Opening Greeting handler (or whatever handler or subscriber you want this to be on) to route the call the the handler you just created.
