Agents unable to logon to ICD 3.0

We are setting up a call center using CallManager 3.22c and ICD 3.01b. We are able to log in a user one time however if you log out and try to log in again the user receives the following error "error trying to monitor device xxxx (the device extension shows).

Any help is appreciated.
