Cisco IP Phone auto forwards itself.

I have a few instances where a phone will appear to forward itself. The icon for forwarding appears, but it does not have the text "Forwarded to Voicemail" and is not forwarded in CM. It is confusing for end users. Does anyone else have any similar experiences?
