Problem starting up an ad-hoc conference with SW CFB in CUCM 11.5, with 7911 IP Phones

When trying to add the 3rd member into the SW CFB, the MOH doesn't stop in the 1st phone and the Conference doesn't start up.

What do we need ?

1) What kind of trace logs is necessary to sort this problem up ?

2) Which are the common reasons for the problem happening ? Codec ? What else ?

You'll need CUCM and IPVMS traces to find out what's going on, most common issues are codecs/BW related.
