Sorry, I don't have CallManager experience, but I am reading a book.
What does Publisher hold exactly? My understanding is TFTP server is holding phone configuration files, not Publisher?
Also about route pattern wildcards, I don't understand ? and +
Eg, 91x?, which can route 91 through 919.....9, what is this? how many 9s?
91x+,which can route 9100 through 919...9, how to explain it, also how many 9s.

Publisher refers to the database behavior. The publisher is the database server which is the authoritative, read/write SQL database which holds the configuration for pretty much the entire system... The TFTP server actually accesses and reads from the SQL database in order to write the configuration files.
The ? wildcard is exactly the same as the ! wildcard, except that ! means any number of digits at least 1 in length. ? means any number of digits at least 0 in length.
The + wildcard means the previous digit or wildcard repeated one or more times. For example,
9! means match 9 followed by 1 or more digits
9? means match 9 followed by 0 or more digits
9+ means match 99 or 999 or 9999 or 99999 or 999999 and so on...
