All,
Can I arrange to send a call directly to hold when the destination line is currently busy?
This is intended to be used at reception, rather than letting the line ring for an extended period (as though we don't care for the call) or sending it to voicemail (which just consumes more of the receptionist's time) it would be great to play them the hold music.
Possibly even hold music for x minutes and then voicemail?

Yes you can. You will have to install IPCC Express to do it.

In CCM 4.1 attendant console can be configured to queue calls when all lines are busy, no answer.

That's positive. You can queue the calls and you can see it in the AC Console Application.
