The easiest way to fix the replication issue is outlined in this document:
http://www.cisco.com/en/US/products/sw/voicesw/ps556/products_tech_note09186a00801e7ddf.shtml
As it says in the document if you can't find the dblhelper program on your CallManager server you'll have to open a TAC case to get it... Also I've noticed that if you install the "upgrade assistant" packages that Cisco release these sometimes install dblhelper to your system..
The SQL SP3a upgrade can be downloaded from:
http://www.cisco.com/cgi-bin/tablebuild.pl/cmva-3des
And is called SQL2K-ServicePack3.1-0-5.exe...

Make sure you carefully read the Cisco CallManager 4.0(1) Service Release 2a: Release Notes v.1. There is a lot of valuable information there. Also make sure you read the SQL Server 2000 Service Pack 3a release notes. I believe SQL SP3a is required for 4.0(1) so if you reinstall it now you may hose SQL altogether according to the SQL SP3a release notes:
Note: If you have successfully installed SQL2K SP3, SP3a or MS03-031 on a server, do not install this upgrade (SQL2K-ServicePack3.1-0-5.exe). Installing SQL2K-ServicePack3.1-0-5.exe on a server that already has a SQL post-SP3 hotfix like MS03-031 installed will cause SQL to become unstable. If you want to confirm the SQL version prior to installing this upgrade, run Select @@Version in Query Analyzer. The result should be Less Than 8.00.760. If the result is equal to or greater than 8.00.760, do not run this upgrade.
It is extremely important to ALWAYS read the release notes BEFORE upgrading or installing ANYTHING on CallManager. No matter how small you may think it is, if you are installing or updating anything on CallManager read the release notes first! :)
