Reason codes were not supported until CallManger 3.1(1). You need that first.

Can we use Unity 2.46 together with CCM 3.1.1 ?

You can if you're using the correct TSP.

To answer your first question, you could do supervised transfers from Unity. Unity can detect busy phones (and then play a busy greeting) when doing supervised transfers regardless of the CCM/Unity/TSP version matrix.
