DS-C9124-K9 Lost Username & password Document

Hi.

I have a MDS 9000 series switch, DS-C9124-K9, for which I made a document that had username IP address and Password everything but now when I need to make some changes inside the device I can't find the document and hence I can't login into my device. Please help me in gaining access to the configuration part inside the device as I can't login. Any help will be deeply appreciated. Thanks in advance.
