Formal Design Documentation?

Our Engineering team is prep'ing to perform a logical redesign of our network. I am in ops and have been tasked with forming the documentation requirements. I am curious if there are recommended network design documents (standardized?) I might review prior to making such requisites?

I'm not aware of any standards, but what I've done at a high level in redesign documents is:

Introduction (Overview of what and why)

Design Specifics (Grave detail about each change and the reasons behind it)

Implementation Plan (The "How")
