Lost Carrier in AS5400

I have AS5400 with 8 E1 card and 276 modem.

I have facing serious disconnection problem, when I saw the disconnections caused in radius log it shows "lost carrier" and "admin reset" and "user rest" mostly the lost carrier is come frequently.

I also have a problem when connecting a user to AS5400 user get error 650 most of the time.

Can you tell me why this happen and how to resolve it.

Thanks for your reply,The E1 PRI is not down only some users get disconnect and the cause of disconnect is Lost of Carrier.
