can you paste outputs of

sh ver

sh crashdump 1

dir bootflash:

sh env 1 (if you are running 5.x or 4.x) or

sh nvramenv 1(if you are running 6.x and higher)

sh ver

From the sh crashdump 1 output you emailed me, I see

Switch_B> (enable) sh crashdump1 1

Version and date:

Old crashdump format or invalid Version and Date <<--

The reset cause is 20 in sh nvramenv 1. This normally indicates some kind of a power failure. Make sure the power to the switch/UPS is stable.

Try doing a "show test switch memory detail" and see if any of the SRAM is coming up faulty . With the version you are running there is continous background test that are run , if the tests see a problem it will automatically reboot the switch and attempt to take the faulty sram segments off and it will run in a deteriorated state until you can change the supervisor . You should see a message in the logs if it is doing this . Do a show logging buffer 1000 and see if anything unusual shows up .
