How to extract a report of the most accessed host (by IP address and Hostname)

I would apreciate if anybody can tell me how can I create reports:

1 - About the "TOP 10 WEB" IP addresses most accessed by my customers.

2 - About the "TOP 10" (in traffic volume) web hosts (by IP address destination or hostname destinations) accessed by my customers.
