Cisco 5508 name changes without interaction

I have 3 Cisco 5508 Controllers and all 3 are setup with a redundant spare (HA). All 3 have unique names and IP addresses but I am having issues with one controller in that it's name that it was originally configured with will periodically change without any interaction and it ends up with the same name as one of the other controllers (always the same one) which cause issues.The version on the controllers is 7.4.121.0 and Prime is running 2.0

Could Prime be somehow changing the controller name and what would cause it to do that? Any suggestions is greatly appreciated

Try this:  

1.  Change the name of the WLC; 

2.  Save the config; 

3.  Reboot the WLC; 

4.  During the reboot, go to PI and remove/delete the WLC; 

5.  Once the WLC comes boots up, add back to PI.
