There must be port loopback test failed on those ports. You can display results via show system health module # command. There are other show logging onboard exception-log command to see more detail

It is most likely a hardware problem. Try reseating the switch module in bladecenter bay If does not solve problem you need a CRU.

Dear Nick,

How did you solve your problem? It was a hardware or software (a NX-OS bug) problem?

I´m with the same situation, I have a N4K-4001I-XPX with NX-OS 4.1.2.E1.1f and the log is the same, not for all ports at same time, but when a loopback test fail for at least one port, all blade servers lost the connection accross the etherchannel. The other port menbers of etherchannel remain with the up status.

Thanks Krunal, we replaced the switch and upgrade NX-OS to 1g and it's normal till now.
