connect two switch with fiber optic converter

How to connect two 2960 switch in different location through FO cable with converter.

We used FO converter to rj45 between the switches and connect the rj45 port in each switches but the link fail from time to time and the port disabled.

Any solution for that error.

If the port(s) went into error disable, what is the output to the command "sh int status error"?
