Today I opened up a new switch and did some basic configuration of VLANs and then sent the "reload" command. The
switch never actually rebooted that I saw, just began the repetitive message of "BOOT FAIL" down the screen. I read
several entries on this forum that stated this was a known bug issue for the "reload" command on this switch. Unlike the
other posts I have found, I have been unable to get the switch to boot normally. I have attempted the hard boot by removing
power and I have attempted to hold the mode button. But nothing gets the switch back online. I cannot tell you the IOS
version as I never looked at it prior, which I am now kicking myself for.
Please advise if you think you have a solution.

Since this is brand new switch with issues, open a ticket with TAC and RMA it.

I did call TAC and they were quite rude about it, I did open a case, waiting for answers.

Larry,
If they are rude and not responsive you can ask to talk to the shift supervisor.
Also, make sure your ticket has priority 1 since the switch is not working at all if not sometimes they can take a long time to get back to you.

The set it to priority 3 and would not let me elevate it.

I just had the same issue on a production switch in a stack configuration.
I was adding a new vlan and got following message
%Error exiting config-vlan mode 47: NV storage failure
also found this in the logs
%SW_VLAN-4-IFS_FAILURE: VLAN manager encountered file operation error: call = ifs_open/write / file = flash:/vlan.dat / code = 22 (Invalid argument) / bytes transfered = 0
This turned out to be the flash on switch 3 was not available writing to the vlan.dat file for that switch.
opened a TAC call and TAC asked me to reload to switch 3 to see if the flash would come back after the reload.
sounded simple enough!!
After typing reload slot 3, I got the boot fail loop
had to get the switch replaced.
