3850 Stack IOS not staying on reboot

I have followed these instructions:

When a reboot is done, only one switch stays with the correct updated IOS.

The other switch only comes up with "switch" prompt and I need to run command :

boot flash:packages.config , then the switch boots back up and is in stack.

 

While switch that will not boot correctly is sitting at "switch:" prompt, the other switch which booted correctly shows this when I issue command "show switch"

If I power off switches again, only one comes up properly, and same thing happens all over again.

The default config register for a 3850 is 0x102.  Check that setting.

 

Anything interesting appear in the log when it fails to boot up?

I will need to console in and check what I see in the logs.
