C2960X-48TS-L satck member stuck at BOOT FAIL

I have 5 C2960X-48TS-L switches connected to the stack 1>2>3>4>5>1, when switches 2 and 5 have power issue switches 3 and 4 disconnect from sw1 and get stuck at "BOOT FAIL" and the SYST led is solid blue. I had to remove power cable and plugin it again to boot correctly, the remaining switches (1,2,5) are booting correctly.

All switches have this image c2960x-universalk9-mz.150-2.EX4, any solutions?

10G/1G interfaces on Catalyst 2960X does not link up after reload or SFP OIR.
10G/1G SFP shows "unknown" in "show interface status".

Conditions:
Issue is seen in Catalyst 2960X switches, running 15.0(2)EX4.
This defect is NOT applicable to earlier or later EX releases.
Workaround:
Reload the switch to recover from this condition.
More Info:
EX3, 4, and 5 releases affected by CSCur56395 can also have symptoms similar to the ones described in this defect. To address both defects, upgrade the switch to 15.0(2a)EX5 or later release.
