Cisco Catalyst 6509-E Switch diagnostics error detected

The Cisco Catalyst 6509-E is equipped with a WS-X6816-10GE module.
Four of the modules have been switched to the GX2-31192-LRC SFP Transceiver err-disable state.

The error log is as follows:

The same symptoms occur when replacing with a new GX2-31192-LRC SFP Transceiver.

What could be the cause?

The line card looks to be cooked. 

Re-seat the line card and post the "sh logs" after the re-seat.  It will determine if the line card has failed POST or not.
