Hi,

even we were faced the same problem and we have some workaround for that and we got 100% success too.

1.Upgrade the new IOS into the switch

2.Delete the old IOS and run "squeeze" command, remove the current boot sequence by "no boot system flash bootflash:cat4000-i5s-mz.121-19.EW1.bin

3.set the boot sequence for the new IOS by boot system flash bootflash:cat4000-i5k91s-mz.122-25.EWA14.bin

4.Upload the old IOS for saferside and set the old IOS as secondary boot sequence

5.reload your switch .

we got success and we are following the same process for the same swithc C4507 .

Hope u will get success for this IOS upload process.

You need to change your config register value from 0x2101 to 0x2102.

0x2101 tells the switch to simply boot from the first image found in bootflash so it boots 12.1(19).

0x2102 tells the switch to look at the boot system statments you have configured and try to boot the image that is first in the list ie. 12.2(25) in your case.

Alternatively you can simly delete the old 12.1(19) image from bootflash and leave your config register as 0x2101 but i would check the new image can load first.
