Fake switches or something else wrong?

Hi 
We purchase 3 series Switch 2960x-fps-l last year, a series  has high failure rate , when Update IOS to "universalk9-mz.152-4.E6", Switch dont work and i see . you can see "" attached pic.

we open box of switch and see a chip attached that. you can see it in "Chip" pic, you can see a chip with blue wire

we open a Switch in a series that correct work when update ios and see a different boad . "OK Sw" pic show this

Third  Series update ios and see again "error message" pic , then open box of Sw . we see board similar first series , bot it not has chip and appear ok status ,for example you can see color board and cooling sink

Did you purchase them from an authorized reseller?

There is a trade embargo in Iran.  

Take a picture and attach the following: 

1.  Serial Number; and 

2.  Model number. 

The pictures should be adequate to identify if the component is fake or not.

The recommendation is to call TAC.  There are some known hardware issues which cause the software to give this error.  They should replace the switch.
