Hi All,
We have a 6509-E chassis with two SUP720-3B sups. A card in slot 4 (WS-X6748-GE-TX ) showed a minor error some days back. Same was observed in the active SUP too(we tried force switchover too). We replaced the slot 4 module after which the minor error disappeared, but after some days all the ports of the module started getting reset, at the same time. This problem gets repeated irregularly, around 2 times in an hour. The Rxerrrors against the active SUP in the 'show fabric channel-counter' also get incremented after each flap. The IOS we are using is s72033-adventerprisek9_wan-mz.122-33.SXI3.bin.

Can you provide the "sh log" of before, during and after the blades reset?
There's a chance that you could be looking at a back-plane issue of the chassis, however, let's take things one-step-at-a-time. Have you tried to upgrade the IOS to either SXI4a or SXI5?
