POE Power Loss on CAT4500E Card: WS-X4748-RJ45V+E

Running 4507E and 4510E's with SUP 7E. Current code Level is  03.06.06.E   I have been running into a issue with losing POE Power on line Cards: WS-X4748-RJ45V+E . The card fails with the following error: %C4K_ETHPOE-4-SUPPLYFAULT: Module 1 POE controller has been shutdown because of excessive power usage.  The card will not provide POE power even after hw-module reset command and de-insert/re-insert card.  I also run line cards WS-X4648-RJ45V+E in the same switches with NO issues.  I need to replace the card (Via RMA) to resolve the issue. 

Currently working with Cisco TAC to see if the following CSC bugs (CSCuz3842, CSCuz42775, and CSCug74475 fit our issue. The Switches and cards were all installed in 2012.  Thinking we have some kind a bad batch issue. 

Any thoughts or ideas? If anyone else has into issue any information would be greatly appreciated.

This is not related but read Upgrading the System Software which states, "If you are upgrading to Cisco IOS XE 3.6.0E and using Supervisor Engine 7-E or 7L-E, you must use ROMMON version 15.0(1r)SG10 or a higher version (if available)."

Can you post the complete output to the command "sh inventory | i PID: PID: WS-X4648-RJ45V+E".

Here is the Show Inventory taken from the Show Tech Support prior to replacing the card.

NAME: "Linecard(slot 3)", DESCR: "10/100/1000BaseT (RJ45)+V E Series with 48 10/100/1000 baseT Premium PoE ports (Cisco/IEEE)"
PID: WS-X4748-RJ45V+E , VID: V03 , SN: CAT1726L0DX

"V03". 

This means the hardware bug applies to the line card.  TAC should arrange for an RMA (and hopefully the replacement line card isn't one of those affected by the "bug").
