Had a look at this document, particularly the "Manual Software Upgrade Process" section and I am "not impressed".  There are a number of important steps missing:

  1. What method to verify if the IOS copied isn't corrupt?
  2. Since Cisco has been (regularly) changing the boot variable string, what is the correct syntax to configure the boot variable string?
  3. Config-registry

The steps that we have came up with is, by far, better.

Agreed, in this Fast world changes happens every few months, I have given the document for high level reference. also suggested to read the release notes also which covers ROMMON Upgrade.

Ultimately I will be opening a TAC case and having them explicitly tell me the correct method and steps for this upgrade.

Read the Release Notes very carefully. It is NOT POSSIBLE to go to 3.8.X without ROMmon upgrade FIRST.
If the ROMmon is not at a certain level, the supervisor card will "reject" the IOS. And if the old IOS was deleted then the supervisor cards will boot into ROMmon.
