SM-X-ES3-16-P OUT OF SERVICE

I'm have an SM-X-ES3-16-P Etherswitch Service Module that I installed into a 4331 Router.  When I do sh platform it is "out of service".  I've tried just about everything, but cannot get the module online.  I put a known good SM-X-ES3-16-P module in the router and it worked fine, so I'm assuming it's the module that could be bad.  Any troubleshooting tips would be greatly appreciated.

SM-X-ES3-16-P is only supported on the 4451-X.

Thanks for the reply, but we have using them for over 4 years with our 4331's and it has just recently become an issue.  The SM-X-16p are compatible according to this link...https://www.cisco.com/c/en/us/products/routers/4000-series-integrated-services-routers-isr/relevant-interfaces-and-modules.html.   I believe it's a glitch in the switch module or a bad configuration command  because I can take a known good switch module and it will work in the router.  

Console into the router and reboot the router. 
Post the entire boot-up process.
