This is way to vague a question and it all depends on the issue and it's urgency.

For A. If you have an issue that is not urgent and you had time you could research it and learn the resolve, but if it is urgent and you do not know it then you should escalate right away.

For B. If you are working an issue that is important but something comes up that maybe only you have the knowledge in you may need to delegate/escalate the current issue so you can work on the new issue.

For C. When you gathered all information that is when you start troubleshooting.

For D. It may be a very urgent issue and if after 5 minutes you can't find a resolve you may need to escalate or it could be low impact and maybe you can work it for many hours before there is a need to escalate. No real set rule here.

Hi

It could be C and/or D, because you need to collect all the information related to the situation, topology, affected tools, TTL, traceroutes, ping, if it is partial or full impact, etc. to analyze where the problem is or to escalate it to other department. And if it is a critical service and you are unable to fix it after a certain time it could be escalated to the next level or vendor.
