 - Try full factory defaults reset with none-user configuration, check if this helps.

I already did, just like what it instructed on the link for the factory reset but sadly, when i press any key to type the "flash_init" for the step two, it would input any.

POST: MA BIST : Begin***ERR: S88G MDIO Read - Address is not decoded.

You having some issue here as mentioned above.

is this working Router and failed or a new one?

1. Power down the switch (pull the cable).

2. Apply power to the switch and force the switch to boot into ROMmon.

3. Use IOS version 15.2(4)E6. - For testing(make sure it stable and working) Download the BIN file (from the Cisco website) and copy to USB flash.

4. Force the switch to boot this IOS using the command "boot usbflash0:IOS_filename.bin".

Test all working.

Then format the Flash:

and copy from USB flash to flash - check the boot variable and wr and test and advise.

It is brandnew, and just for clarification, do i need new license if I upgrade the IOS?

Or is there anything I need to be careful with not to do(or to do) so that it won't add another problem.

no license is bound to the chassis , upgrading software wont effect it
if this is brand new and has not worked before , if the software upgrade does not work , return the switch

if no usb you can use xmoden either , this guide shows you what you will see as the upgrade occurs , its same if its by usb just another transfer method to get the software onto the switch

https://www.freeccnaworkbook.com/workbooks/ccna/recovering-a-corrupt-cisco-ios-image-on-a-catalyst-switch

Sadly upon entering the Switch Rom mode, i can't type anything and I even tried waiting for 2hours to see if there is any result, but to no avail still nothing happens. But whatever I type would reflect once I remove the power from the switch. Is there any way to do the a IOS recovery?

You need to be able to type in CLi mode to do the IOS recovery from the switch prompt , try rebooting it if not already done, try change the cables too if possible , if this is a brand new switch you should be able return it under DOA or RMA in first 3 months warranty even without full support , the fact its getting to switch mode would suggest its working at some level

It woeks for like almost a month but there is a power outage(in our area) all of a sudden and upon booting it up again this what happen. I tried contacting the TAC

Unfortunately, this product does not appear to be under contract for technical support. In order to receive technical support, a contract would need to be purchased through a Cisco partner/reseller.

I was already able to type on CLI, I already added the new .bin file "c2960x-universalk9-mz.152-4.E6.bin"

and boot the file but still boot fail.

The error below catches my attention, and I am wondering is this another factor why it is still boot fail despite changing the IOS? Or i really need to format the flash so that it works (because I just added the new .bin file and not format it)

Looks like Switch went bad, raise TAC for RMA.

looking at an older post that is not a good sign if its failing POST and may be dead , see Leos post on this alert previously ,needs to be replaced , without support you take the hit unfortunately

https://208.74.205.244/t5/network-management/catalyst-2960-x-series-booting-loop/td-p/3900396

Since it is already more than 30 days from us and no SNTC, it means this thing is = to garbage? No other way get it replace or atleast replace the defective component.

Any advise would be really appreciated.

its a power on self test failure so something looks to be gone faulty , if software has not fixed it , it needs to go to Cisco

With no support with Cisco there is other companies out there depending on your location , that will fix broken Cisco devices out of support or at least attempt too , you could try one of them as another option

Just an example but i have seen these in few different places , better than it being binned

https://www.comtek.co.uk/hardware-repairs/cisco-repair/

Which/what part of the switch is failing? Is it possible to narrow it down?

By the way i really appreciate your help.
