Dear Experts;
I am very new to VCS and Cisco TP.
We are now implementing Cisco Tele presence with VCS-C, VCS-E TMS,TCS,MCU and End points with Jabber in One setup.
and in another setup CUCM 10.5,UCCX 10.5 IM&P,Jabber with some 10 agents.
Now the question is In our same building in 2nd floor we have one EX90 and in 5th Floor one EX90 and over LAN we can make video calls using IP address.
In the same way is it possible to make a video call between 2 EX90 devices(Both having Public IP) present in different location in the same city over Internet without the involvement of VCS-C and VCS-E.

Yes, but by leaving these systems out in the wild with public IP addresses, you are leaving them vulnerable to a number of issues; i.e. see http://www.videonationsltd.co.uk/2014/11/h-323-cisco-spam-calls/

The above deals with H.323 calls, on top of that you'll encounter similar issues using SIP where the systems will be scanned by tools like SIPVicious

For EX90 you reqiured one license and r u calling from TMS or direct.

As you have a VCS-E, which is most likely connected to the internet, you could also
register the EX90 to that VCS. The systems can be on a public ip (when not firewalled you
will face the issues Jens listed there as well) or simply behind a generic NAT.
Thats how I would deploy it if you can not put them in your inside network.
If you also expect TMS management for outside endpoits you will need to do some
work there as well.
