Unity Connection - Line fwd on FNA, FBNA won't go to *xxxx mailbox

Scenario

I have 4 phones for one team. This is not a hunt group, all team members have their own individual DID. However, the manager wants all of the vm to go to one single vm box. 

Team member 1 - ext.1134 - main mailbox <--- works fine

Team member 2 - ext.2234 - Fwd settings vm to *1134 <--- works fine

Team member 3 - ext.3334 - Fwd settings vm to *1134 <--- works fine

Team member 4 - ext.4434 - Fwd settings vm to *1134 <--- This one will not fwd to *1134 instead goes to Unity treatment "Hello cisco unity messaging system, from a touchtone phone.........ect" 

 

I've even tried adding as an alternate extension to 1134 vm box and still won't work.

 

Any help is greatly appreciated.

Is there a voicemail profile attached to DN 4434?

No it was removed because the manager said he no longer wanted voicemail to go to that box and instead re-route to *1134 like the other two. They work fine but this one just goes to messaging mary every time.

Do the other DN's that do go to a VM box have a voicemail profile? It is a bit difficult to speculate too much without seeing the whole system. Another possible problem could be if the VM box is tied to last forwarding station instead of the original called party. I think that is a CUCM service parameter, but I don't recall clearly. I haven't had to edit that in a quite some time.

The Line settings for Team Member #4 Device (ensure that the voicemail option is unchecked for all call forward types except the call forward all ). Set the destination as Extension #1134 and make sure to select the appropriate Calling Search Space (CSS).
