we were not receiving any messages etc.. restarted Unity and it was working. the following error message was in app log:

The defect CSCef24576 matches this event message and is resolved in the SR1 to be posted to CCO this week. There is a pre-SR1 ES 20 avaialable for 4.0(4) which also resolves this behavior.

To be sure, either the ES20 if you need it right away or the SR1 as soon as it is available will be necessary to resolve the behavior.
Once the condition is hit (as shown by this event), system availability could be impacted at any time.
