I agree that the information about gateway codes is not very clear, you can however check the following snippet from the PCA guide with the text in bold providing some details about how gateway codes come into picture
The following table provides the default dial plan values.

 Note |
Cisco Prime Collaboration classifies the call as Toll Free if the toll-free code is defined in the dial plan that is assigned to the cluster. |
