Unity4.2(1)--CCM5.0(4)--VG(FXO)--VG(FXS)--Analog phone
1. Analog phone able to dial to IP phone and works okay
2. Analog phone get an error when dial to Unity call handler with the error greeting 'I did not recognize as an valid entry......'. follows the Unity log.
why extra DTMF added when the call coming from FXO?

From that trace it looks like Unity is receiving four "5" digits from CCM. If that is different than what was sent from the analog phone then you'll probably need to look at more detailed traces to determine where the digits are coming from. Unity is just reporting and acting on the digits it sees.

You can cun a debug vtsp on the gateway to see if duplicate digits are being sent.
Also, you can do a pcm trace and capture the actual tones. I had this once before and TAC decoded the pcm traces and sent me the .wavs of the tones. The tones were showing up as garbled which the g/w broke into two or more separate tones which were passed on to Unity
I can't, however, recall the resolution...possibly a bad dsp or an issue on the telco side.
