C220 M3: firmware update-all fails with FPGA errors in CIMC log

Hi all!

I've got a C220 M3 server I am trying to solve an issue where the front panel disk LEDs are always on, irrespective of status.

The firmware upgrade process is failing. I have the latest 3.0.4s bundle installed, however I can't seem to install all chassis firmware:

C220 /chassis/firmware # update-all
Starting firmware update process, this will take a while. Check status using show command
C220 /chassis/firmware # show detail
Firmware Update Required. Please run update-all command..

In the Cisco IMC log in the CIMC GUI, I see the following:

BMC:run-pdate:
 ../../src/runupdate.c:739: Chip Version: 31553032
 cpld.c:97: CPLD version compare
cpld.c:101: Chip and file version match.
 ../../src/runupdate.c:766: Skipping device.
 ../../src/hw_i2c.c:44: Error: I2C Read IOCTL Fail: (0x0000002B)
fpga.c:35: Error: Failed to access I2C device
fpga.c:79: Error: FPGA unable to get the image file information
../../src/runupdate.c:710: Error: Failed to get image file version: FPGA
 ../../src/runupdate.c:930:#012 Run-update Process Complete. rc: 2#012

Am I missing something, or is it likely I have faulty hardware?
