ESXi 6.5 fails to see the LUN on the SAN UCS mini

Driver 1.6.0.28

 

i'm not sure driver version for ESXI 6.5 is correct.

Are you booting from SAN? If not, you may need to allow the initiators to access the target on the SAN storage side and also correct your fnic drivers...
