HyperFlex upgrade failing with "Configure networking script generated an exception: Failed to login controller VM"

I am upgrading an HX240 cluster from 2.6(1a) to 2.6(1d), and the upgrade has completed successfully on 2 of the 4 nodes. One node fails after the bootstrap, VIB upgrade and reboot steps with the above error. I have re-attempted the upgrade multiple times to no avail. All nodes were initially installed at the same time, and there should not be any difference in the credentials between nodes. Is there any way to confirm what credentials it's trying to use, and whether the credentials match between this node and others?

Passwords need to be the same for the following:
- SCVMs (same password for each SCVM, can be different from ESXi password)
- ESXi (same password for each ESXi host, can be different from SCVM password)

SSH to each SCVM and ESXi to confirm the passwords match, respectively

If passwords are matching and still having this problem, open a case with Cisco TAC to troubleshoot
