6100 Series UCS.
Cisco UCS Manager - 2.0(1w)
We use TACAS for authentication. All is good, until recently when tryong to connect to KVM getting:
Tried resetting KVM on blade, and have cleared stail logins to UCS.
Anyone run into and gotten past this?

Craig,
I assume you have logged into UCSM management or KVM Manager and are now launching the KVM for the unique blade or service profile. Test the following:
- Confirm are using the JAVA version appropriate for 2.0(1w)
- Flush the Java cache
- Flush the Browser cache.
Is this issue the same for all blades. Is this issue the same from other workstation launching the KVM.

I should say it "is not" all good. Started a TAC case on this, will report back. All is good, nothing has changed and KVM login is failing. Even logging in as local admin does not make a difference. Did a "recover server" and restarted the KVM, but nothing.

I had something similar a while ago and it turned out to be overlapping management ips across different UCS domains.
Here is a quick way to verify if anyone runs into this in the future:
http://x.x.x.x/kvm.zip (where x.x.x.x is the UCSM ip address)
Unzip kvm.zip
Execute launchkvm.bat
Insert the blade's CIMC managemt IP address and UCS credentials
If the KVM loads, check to make sure it is the expected server.
