CUCM V9.1.2 & Revolabs FLX2 VoIP

Followed the setup guide for Revolabs FLX 2 on CUCM to the letter but having no luck at all getting it to register. Registration and IP address in CUCM both show as "Unknown" Has anyone come across these errors and fixed them?
